The Waldwin Groups Healthcare plan is offered by BCBSMA and includes a large network of BCBS providers. To utilize the benefits of the HMO, you must coordinate your care through your Primary Care Physician (PCP), use the referral process and use the HMO Blue network of providers.

The HMO will cover:

• Office visits, routine care, routine GYN (including lab tests) and routine eye exams, with a $15 co-payment.

• Prescription drugs with a co-pays of $10/$25/$45 for prescription drugs purchased in the pharmacy (30 day supply).

• $10/$25/$45 for prescriptions drugs purchased through the BCBSMA mail order plan (90 day supply).

• For eligible medical expenses in excess of the HMO’s copayments and Deductibles, BCBSMA will cover 100% for services provided in-network.
